Abstract

Neuromyelitis optica spectrum disorder (NMOSD) is a rare and often aggressive autoimmune disease in which the immune system attacks the optic nerves and spinal cord, frequently causing severe vision loss and paralysis with each relapse. Most patients are managed with long-term immune-suppressing medications aimed at preventing further attacks, but these drugs don't cure the disease, and relapses can still break through. A cure, or anything close to one, has not been part of the conversation for most NMOSD patients.

A case report published in the journal Med (a Cell Press publication) on June 15, 2026, describes two patients with severe, treatment-resistant NMOSD who received allogeneic hematopoietic stem cell transplants, meaning they were given a donor's stem cells rather than their own. Both patients have now remained completely relapse-free for more than 15 years, one for 15 years and one for 16, without taking any ongoing immune-suppressing medication. That is the longest documented follow-up of its kind for this disease.

What makes the case genuinely striking isn't just the length of remission, it's what happened at the biological level. The damaging antibodies that drive NMOSD, called AQP4-IgG, became completely undetectable in both patients and stayed that way throughout the entire follow-up period. Neither patient developed graft-versus-host disease, a common and serious complication of donor stem cell transplants, and both maintained full donor chimerism, meaning their new donor-derived immune system fully took over without their original immune system re-establishing itself.

Researchers believe the new donor immune systems developed an expanded population of regulatory T cells, the immune system's own built-in brakes, which may explain why the disease hasn't returned even without ongoing medication. That's a meaningfully different mechanism than simply suppressing the immune system indefinitely, it points toward the immune system relearning tolerance rather than being held in check by drugs.

It's important to be realistic about what a two-patient case report can and can't tell us. This is not a clinical trial, and allogeneic stem cell transplant is a serious, high-risk procedure with its own significant complications, it is absolutely not something undertaken lightly or for milder cases. But researchers say these results are strong enough to justify designing a larger, more formal study to understand who might benefit and how safely this approach could be offered more broadly.

For the NMOSD community, a disease where relapses can mean permanent vision or mobility loss, even early, cautious evidence of a potential path to lasting remission is worth paying attention to, and worth discussing with a specialist familiar with transplant-based approaches if your disease has been especially severe or treatment-resistant.
