Perhaps most notable to patients and clinicians, the company reported early signs that some responses held up even after people stopped taking the drug, including one patient who regrew a full head of hair.

Bempikibart works differently than the JAK inhibitors already on the market. It is a fully human antibody that blocks two separate signaling pathways, interleukin-7 (IL-7) and thymic stromal lymphopoietin (TSLP), both of which appear to push T cells to mistakenly attack hair follicles. Because it targets a different part of the immune pathway, researchers hope it could work for patients who do not respond well to JAK inhibitors, or offer another option for patients who want to avoid the class's known side effects.

The trial also tracked safety and immunogenicity. Q32 Bio reported that bempikibart showed a favorable pharmacokinetic and pharmacodynamic profile in Part B, with negligible anti-drug antibody formation, a reassuring sign for a biologic drug meant to be used over the long term.

Alopecia areata affects millions of people worldwide and can range from small bald patches to complete loss of scalp and body hair. While it is not physically dangerous, it can take a significant emotional and social toll, and treatment options have been limited until the recent arrival of JAK inhibitors. A second, mechanistically distinct option like bempikibart could give patients and doctors more room to individualize treatment.

Q32 Bio said it is targeting the first half of 2027 to begin a registration-directed program for bempikibart, meaning a pivotal trial intended to support an eventual FDA application. That timeline puts any approval years away, and the current results come from a relatively small trial rather than a large randomized study, so the findings will need to be confirmed in bigger trials. Still, the durability signal after stopping treatment is the kind of result that tends to catch the field's attention, since most current alopecia areata therapies require continuous use to maintain results.
