Bempikibart is designed to work differently than JAK inhibitors. It blocks a receptor for two immune signaling proteins, IL-7 and TSLP, that are thought to help drive the autoimmune attack on hair follicles, rather than broadly suppressing the JAK signaling pathway used throughout the immune system. The hope is that a more targeted mechanism could offer meaningful hair regrowth with a cleaner safety profile than existing options, though that comparison has not yet been tested head-to-head. In the trial, the primary measure of success was the change in SALT score, a standardized tool dermatologists use to estimate what percentage of the scalp has lost hair. Patients treated with bempikibart saw their SALT scores improve by a mean of 35.3% from baseline by week 36, and 40% of patients achieved what researchers call a SALT-20 response, meaning they regrew enough hair to cover at least 80% of the scalp. For a population starting with severe, and in some cases nearly complete, hair loss, regrowing that much hair represents a substantial and visible change.

The company reported that the safety profile remained well tolerated through 36 weeks with no new safety signals compared to earlier data from the trial. Q32 Bio said it plans to hold an end-of-phase 2 meeting with regulators by the end of the year and aims to start a registration-track program in severe and very severe alopecia areata in the first half of 2027. That means a potential approval, if the drug continues to perform well in larger trials, is still likely several years away, but this 36-week durability data is an encouraging sign that the early response seen in the trial is holding up over time rather than fading.
